Students entering into the 7th grade are REQUIRED by the State of Florida to have Tdap vaccination PRIOR to starting 7th grade. The only documentation that is acceptable is a Florida Certificate of Immunization Form 680. This form must be on file with the School Nurse before your child can start 7th grade. Immunizations are free at the Manatee County Health Department. To schedule an appointment at the Health Department, call 941-748-0747 ext. 6656.
