Medicine Pick-Up from Clinic

A message from the RMA clinic:
 
***PLEASE PICK UP MEDICATION FROM THE CLINIC***
 
BY THURSDAY, MAY 27TH
EARLY OUT!
 
Dear Parent/Guardian:
 
The end of the school year is quickly approaching! Please pick up your child’s medication by 1:30 on THURSDAY, May 27, 2021, which is the last day of school for students. Any medication that is not picked up by 1:30 on May 27th will be discarded.
Also, at this time, please pick up a new blank Medication Authorization Form for the next school year (Or you can print it from our website). We request that you ask your pharmacist to give you a second identically labeled container for any prescription medications your student will take at school. This will allow the School Nurse to send these medications on field trips and comply with Florida State laws pertaining to medication storage.
 
Florida State Law requires that:
• New medication forms for both prescription and over-the-counter (OTC) medications must be signed by the provider and parent annually. Orders are good for one year from the date they are written.
• The parent must sign the medication authorization form.
• Medication must remain in the properly labeled pharmacy or original OTC container with a prescription label from the pharmacy.
• Parents or guardians must personally deliver all medication to the to the school health office. Never send ANY medication with your child.
