7th Grade - TDAP Requirement
The State of Florida requires proof of Tdap immunization on all students entering 7th grade in 2024-25. This proof must be on the official State of Florida Form DH-680 which is obtained from your doctor’s office or the Health Department.
It is recommended that you complete this requirement before the end of their 6th-grade year to avoid any risk of your child’s exclusion from school next year.
Once you have received this form, make sure it indicates that Tdap was given and submit it to the school nurse. (must be received prior to the first day of school)
