I affirm that my student is in good standing at his current school. He/She attends school on a consistent basis (no more than 10 absences this year, arrives to school on time, has had no behavior issues and receives grades of D’s or better.
I am aware of the Arts, Communication and Leadership theme of Rowlett Middle Academy and am committed to fulfilling the school’s parental requirements.
I understand the principal of Rowlett Middle Academy may recommend the revocation of choice if you or the student fails to comply with attendance, punctuality and student conduct requirements.

If the number of applications exceed the number of students to be accepted, Rowlett Middle Academy, in accordance with the Florida Law governing Charter Schools, will use a lottery system to determine which students will be accepted. I have read the information in this application and it is my desire and intention to enroll this student at Rowlett Middle Academy. I attest that the information provided is true and accurate to the best of my knowledge. I understand that falsification of information can result in loss of eligibility. If my child is accepted, I will need to complete commitment forms and a Parent/Student Contract within approximately 2 weeks, or acceptance may be subject to cancellation.
